The RN Case Manager will create, implement, and monitor treatment plans to assist patients/members in meeting their healthcare goals. The role involves assessing the member's health status, care coordination needs, and providing interdisciplinary care options. The Case Manager will work with patients, family, physicians, and community resources to identify alternative treatment plans and promote optimum value for the client and reimbursement source.
