Aveanna isn’t just a provider of compassionate homecare to children and adults. <strong>We are a national leader</strong>.</p><p>    <br />Essential Job Functions<br><br>Assume responsibility in coordinating care to assigned patients, establishing a goal directed care plan from admission to discharge including a comprehensive, ongoing assessment of patients’ needs.<br />Perform on-site supervisory visits to assess patient, family, environment, and clinical caregivers and complete timely follow-up documentation.<br />Availability to take on-call duties as assigned, demonstrating a commitment to providing timely and responsive patient care whenever needed. <br />Uphold and promote adherence to CHAP (Community Health Accreditation Program) standards, ensuring the delivery of high-quality and compliant nursing services.<br />Contribute to retention initiatives by fostering a supportive, engaging work environment, and providing valuable insights to enhance caregiver satisfaction and longevity within the organization.<br />Ensure availability and proper operation of necessary equipment and supplies related to patient care.<br />Ability to assess patients and provide direct patient care as needed.<br />Promote and manage expectations and satisfaction with internal and external customers.<br />Evaluate the quality and effectiveness of nursing services,<br />Analyze and report clinical data and relevant findings that could provide opportunities to improve patient outcomes or mitigate risk.<br />Provide nursing updates and obtain re-authorization for continued care.<br />Provide ongoing supervision, orientation, training, education, and evaluation of clinical caregivers.<br />Adhere to professional practice standards within the organization.<br />Contribute to the advancement of the individual’s professional development. <br />Participate in employment decisions affecting clinical caregivers, including hiring and termination as appropriate.<br />Maintain compliance in accordance with company policies and procedures, laws and regulations, and professional standards within the state of practice.<br />Maintain a professional demeanor consistent with registered nurse standards of practice.<br />Adhere to ethical principles and foster a non-judgmental, non-discriminatory attitude.<br />Participate in after-hours calls, as needed.  <br />Promote an environment of quality and safe patient care.<br />Follows the location chain of command for reporting and communicating essential information.<br><br>Diploma, Associate, or bachelor’s degree in nursing from state accredited RN program<br />Clinical or other work experience as per state and/or federal laws and regulations<br />RN licensure in designated state(s) as appropriate<br />Valid Driver’s license and Acceptable MVR<br><br>Preferences<br><br>RN experience and the knowledge, experience, and ability to effectively administer the private duty program.<br><br>Other Skills / Abilities<br><br>Must always adhere to confidentiality standards and professional boundaries.<br />Knowledge and understanding of compliance with adherence to regulations.<br />Ability to comfortably work with families with limited resources.<br />Quick-thinking and astute decision-making skills.<br />Attention to detail.<br />Time Management<br />Effective problem-solving and conflict resolution<br />Excellent organization and communication skills<br />Ability to remain calm and professional in stressful situations.<br />Strong commitment to clinical excellence<br><br>Physical Requirements<br><br>Must be able to speak, write, read, and understand English.<br />Must be able to travel; company does not provide vehicles or transportation.<br />Occasional lifting, carrying, pushing and pulling of up to 40 pounds.<br />Prolonged walking, standing, bending, kneeling, reaching, twisting.<br />Must be able to sit and climb stairs.<br />Must have visual and hearing acuity.<br />Must have a strong sense of smell and touch.<br />Must be able to sufficiently reposition patients and move equipment without assistance.<br />Must be able to appropriately respond physically and mentally to emergency situations in the home or during transport.<br />Occasional rapid movement and physical agility in response to the spontaneity of children and those patients with neurological impairment<br><br>Environment<br><br>Must be able to function in a wide variety of environments which may involve exposure to allergens and other various conditions.<br />Possible exposure to blood, bodily fluids, and infectious diseases<br><br>Other Duties<br><br>Please note this job description is not designed to cover or contain a comprehensive listing of activities, duties or responsibilities required of the employee for this job.
