The Part-time Interim Licensed Social Worker assumes the lead role in the delivery of psychological, financial, religious, and physical needs of the resident, family members and significant others. The Part-time Interim Licensed Social Worker assures that the resident’s needs are met in accordance with the mission statement, philosophy, policy and procedures of the facility. Promotes a team concept with other departments and disciplines through demonstrating good interpersonal skills. Job Description Maintains constant interaction with other disciplines in the needs of the residents Gather appropriate social information at time of admission Maintains on-going contact with family members Assists with in-home or in-hospital pre-admission assessments as needed Assists residents in psychological, physical, emotional and religious needs Assists residents, family members, significant others with Medicaid application redetermination and Medicare issues Qualifications LSW, LCSW or LICSW license required Completed internship or fieldwork requirements Master's degree preferred IND123
