• Conducting initial health assessments, including condition-specific issues and medications
• Documenting clinical history and activities of daily living
• Evaluating mental health status, cultural and linguistic needs, and caregiver resources
• Collaborating with healthcare professionals, patients, and families to ensure seamless care coordination

To be successful in this role, you'll need:

• An Associate's Degree in Nursing
• An active RN license
• Strong communication and assessment skills
• Ability to work independently in a remote setting
