Job Summary
We're seeking an experienced LVN/LPN Care Management Professional to join our team in a full-time telehealth role. As a key member of our care coordination team, you'll play a vital role in ensuring seamless transitions of care for our patients, facilitating post-discharge goal attainment, and providing compassionate attention and excellent communication to our patients and their families.
Responsibilities
As an LVN/LPN Care Management Professional, you'll be responsible for:
- Completing post-discharge questionnaires and ensuring patients have filled/received their medication(s) and understand how to take their ordered medications
- Providing benefit education and monitoring patients in low CM level for alerts or changes in condition to be transitioned back to RN
- Completing post-discharge calls and required assessments (RAP), medication reconciliation (if within scope of practice), and fall assessment if fall risk identified
- Managing warm transfers from concierge and engagement hub and providing clinical assistance to determine appropriate services and supports due to patient's health needs
- Evaluating health and social indicators, identifying and engaging barriers to achieving optimal patient health, and using discretion to apply strategies to reduce patient risk
- Facilitating overall care coordination with the care team to ensure patients achieve optimal wellness within the confines of their condition(s) and abilities to self-manage
- Coordinating post-discharge meal delivery, assisting with securing DME, and helping to ensure timely physician follow-up
- Understanding Payer/Plan benefits, policies, procedures, and articulating them effectively to providers, patients, and other key personnel
Requirements
To be successful in this role, you'll need:
- 2+ years of LPN or LVN nursing experience
- Active and unrestricted compact LPN or LVN licensure in the state of residence
- Self-motivation, energy, detail-oriented, highly organized, and tech-savvy
- Discharge planning experience and advanced proficiency in Microsoft Word, Excel, and Outlook
- Ability to multitask, prioritize, and effectively adapt to a fast-paced changing environment while providing outstanding care
- Effective verbal and written communication skills
Preferred Qualifications
While not required, we're looking for candidates with:
- Bilingual (Spanish) skills
- Associate's or Bachelor's Degree in Nursing or related field
