Key Responsibilities
- Conduct comprehensive assessments of patients' social and emotional needs, developing personalized plans of care to address these needs.

Essential Qualifications
- Masters or doctoral degree in Social Work from a school accredited by the Council on Social Work Education.
- Current Social Worker licensure in the state of practice, if required by state law or regulation.
- Valid driver's license, auto insurance, and reliable transportation.
- Proof of current CPR certification.
- Minimum of one year of experience as a social worker in a healthcare setting, preferably in home health or hospice care.
