Position Overview
We're looking for a skilled Case Manager RN to work in the field, serving members in Wayne and Macomb Counties. As a Case Manager, you'll play a vital role in assessing, planning, implementing, and coordinating care management activities to address the complex health and social needs of our members. This is a unique opportunity to work independently from home, with 25-50% travel within the designated counties.
Key Responsibilities:
- Conduct comprehensive assessments of members' needs, using clinical tools and data review to identify areas for improvement.
- Develop proactive plans of care to address identified issues, enhancing short-term and long-term outcomes, and promoting overall wellness.
- Apply clinical judgment to incorporate strategies that reduce risk factors and barriers, addressing complex health and social indicators that impact care planning.
- Collaborate with supervisors, providers, and community organizations to overcome barriers, meet goals, and present cases at interdisciplinary case conferences.
- Utilize case management processes in compliance with regulatory and company policies, and motivational interviewing skills to ensure maximum member engagement.
- Maintain accurate and up-to-date records, using effective computer skills and software applications.
Requirements:
- Active Michigan state license as a Registered Nurse.
- Minimum 3+ years of clinical experience, with 2+ years in care management, discharge planning, or home health care coordination.
- Ability to travel 25-50% within Wayne and Macomb Counties.
- Associates Degree or Nursing Diploma required; BSN preferred.
Preferred Qualifications:
- Experience working with Medicaid or waiver populations.
- Bilingual skills in Arabic or Spanish.
- Certified Case Manager (CCM) or equivalent certification.
- Excellent analytical, problem-solving, and communication skills.
- Proficiency in standard corporate software applications, including MS Word, Excel, Outlook, and PowerPoint.
