About the Role: Registered Nurse, Virtual Care - Remote
In this dynamic and rewarding role, you'll have the opportunity to provide telephonic triage, directional patient care advice, and health education to our patients and their families. Key Responsibilities:
- Conduct telephonic triage and provide directional patient care advice to patients and their families, utilizing your clinical expertise and judgment to address a wide range of health-related issues.
- Develop and implement comprehensive care plans that prioritize patient-centered care, health education, and disease management, in collaboration with our interdisciplinary care team.
- Utilize our state-of-the-art technology to monitor patients' vital signs, track health outcomes, and provide remote clinical assessments and interventions as needed.
- Collaborate with on-call providers and other healthcare professionals to ensure seamless care coordination and optimal patient outcomes.
- Document patient interactions, care plans, and outcomes in our electronic health record system, maintaining accurate and up-to-date records that inform our care decisions.
- Participate in quality improvement initiatives and contribute to the development of best practices in telehealth and virtual care, helping us to continuously enhance our services and improve patient outcomes.
Essential Qualifications and Skills:
To succeed in this role, you'll need to possess a unique blend of clinical expertise, technical skills, and interpersonal abilities. We're looking for registered nurses who are:
- Graduates of an accredited nursing program, with an Associate's or Bachelor's degree in Nursing.
- Licensed to practice as a registered nurse in the state of employment, with a Nurse Licensure Compact license or willingness to obtain additional licenses as required.
- Certified in Basic Life Support (BLS) from the American Heart Association or American Red Cross, or willing to obtain certification within the first 90 days of employment.
- Experienced in acute clinical nursing, with a minimum of 2 years of experience preferred, and at least 1 year of experience in telehealth or virtual care.
- Proficient in Microsoft Office Suite products, including Excel, Word, PowerPoint, and Outlook, with the ability to learn and adapt to new technologies and systems.
- Fluent in English, with bilingual proficiency in Spanish or Creole preferred, to support our diverse patient population.
- Able to work independently and as part of a team, with excellent communication, problem-solving, and critical thinking skills.
- Committed to delivering exceptional patient care, with a strong focus on customer service, empathy, and compassion.
