Job Summary

We are seeking an experienced and dedicated Registered Nurse to join our Case Management team as a Remote Registered Nurse - RN - Case Management Specialist. This is a full-time, remote, telehealth role that offers the opportunity to work from the comfort of your own home and make a real difference in the lives of our patients. As a Case Manager RN, you will be responsible for assessing, planning, implementing, and coordinating all case management activities with members to evaluate their medical needs and facilitate their overall wellness.

Key Responsibilities

• Conduct comprehensive assessments to identify the medical needs of our members and develop proactive plans to address their short and long-term outcomes.
• Use clinical tools and information/data review to evaluate members' needs and benefits, and apply clinical judgment to incorporate strategies designed to reduce risk factors and barriers.
• Collaborate with supervisors and other key stakeholders in the member's healthcare to overcome barriers and meet goals and objectives.
• Utilize case management processes in compliance with regulatory and company policies and procedures, and leverage motivational interviewing skills to ensure maximum member engagement.
• Work on behalf of the client and the member's provider to ensure seamless care coordination and communication.

Essential Qualifications

To be successful in this role, you will need to possess the following essential qualifications:

• Be a Registered Nurse with an unrestricted nursing license and multi-state/compact privileges, and have the ability to be licensed in all non-compact states.
• Have a minimum of 3+ years of clinical/nursing practice experience, with 1+ year of medical case management experience, care coordination, or discharge planning.
• Be COVID-vaccinated, as required by our client.
• Have high-speed internet access and the ability to hardwire your computer.
• Be proficient in personal computers, keyboarding, multi-systems navigation, and MS Office Suite applications, with a minimum of 3+ years of experience.

Preferred Qualifications

While not required, the following preferred qualifications will make you a stronger candidate for this role:

• Telephonic case management experience, with excellent communication and interpersonal skills.
• A Bachelor of Science in Nursing (BSN) degree, although an Associate's degree in nursing is the minimum requirement.

Education and Pay Range

We require a minimum of an Associate's degree in nursing, although a BSN is preferred. The typical pay range for this role is $66,575.08 - $142,576.72, depending on experience, education, geography, and other relevant factors. This position is also eligible for a CVS Health bonus, commission, or short-term incentive program, in addition to the base pay range.
