Key Responsibilities
As a Home Health Physical Therapist, your primary responsibilities will include:
- Planning and administering prescribed skilled physical therapy treatment and training for patients suffering from various injuries, illnesses, and functional disabilities to attain the highest level of physical function.
- Testing/screening patients' physical strengths to assist physicians in evaluating patients' levels of function and recording findings to develop or pursue treatment programs and establish measurable training objectives.
- Developing/implementing a conditioning/rehabilitation program consistent with the physician's Plan of Treatment and the overall goals of the patient/rehab team.
- Adjusting treatment as needed to achieve maximum results.
- Confering with physicians and clinical team members to obtain additional patient information and assist in developing, implementing, and revising the therapy treatment program and Plan of Treatment.
- Providing Physical Therapy Assistants and Home Health Aide staff with written instructions/care plans that reflect the current plan of care as related to therapy, supervising/evaluating staff performance.
- Monitoring the appropriate completion of documentation by physical therapy assistants and home health aides/personal care workers as part of supervisory/leadership responsibilities.
- Accurately, promptly, and thoroughly documenting patients' care observations, interventions, and evaluations.
- Assuring that interim (verbal) orders received from physicians and physical therapists are promptly and accurately documented, submitted for physician signature, and implemented.
- Reporting patients' progress to patients' physicians, Clinical Managers, staff, patients, and families.
- Submitting evaluation, treatment plans, progress reports, and discharge summaries to supervisors and care management staff.
Requirements and Qualifications
To be successful in this role, you will need:
Essential Qualifications:
- A degree from an accredited Physical Therapy Program (approved by the APTA).
- Current and unrestricted Physical Therapy licensure.
- Current CPR certification.
- Strong organizational and communication skills.
- A valid driver's license, auto insurance, and reliable transportation.
Preferred Qualifications:
- Minimum of one year of physical therapy experience.
- Home Health experience.
