Conduct patient interviews and explain policies and procedures to patients and significant others.
Assess gastrointestinal, cardiovascular, respiratory, renal, and neurological health status of patients.
Determine mobility, sensory deficits, prostheses use, and skin condition; adjust plan of care as appropriate.
Assess level of pain and pain management; communicate and document patient's physiological health status and plan of care.
Assess patient's psychosocial health status and elicit perceptions of medical/nursing care.
Develop individualized patient outcome statements and establish goals based on assessments.
Coordinate the cost-effective use of supplies, equipment, and medication to achieve expected outcomes.
Document the plan of care and collaborate with physicians and other health team members.
Implement the plan of care and maintain constant vigilance over patient care to ensure safety precautions are followed.
Reassess patients as needed and appropriately revise the plan of care; administer prescribed treatments correctly.
Provide emotional support to patients and significant others; apply scientific principles in performing procedures safely and efficiently.
Communicate effectively with the patient's family or caretakers and participate in planning for discharge.
Evaluate care provided for patient outcomes and document effectiveness of care.
Collaborate with other care team members in planning and carrying out treatment regimens.
Assist the physician during treatments and/or diagnostic procedures; keep the Patient Care Manager informed of changes in patient condition.
Educate patients and significant others regarding drug and food interactions and provide health teaching opportunities.
Requirements
Graduate from an accredited School of Nursing.
Current License to practice as a Registered Professional Nurse in New York State required.
2+ years of critical care or PACU RN experience preferred.
Bachelor's Degree in Nursing preferred; must be enrolled in an accredited BSN program within two years and obtain a BSN Degree within five years of job entry date.
