About the Role: Care Manager RN
We're seeking an experienced Care Manager RN to join our team in Oklahoma as a full-time field-based telecommuter. As a Care Manager RN, you'll be responsible for driving and supporting care management and care coordination activities across the continuum of care. Key Responsibilities:
- Conduct telephonic and/or in-person assessments, planning, execution, and coordination of all care management activities with members to evaluate their clinical and behavioral health needs.
- Develop proactive plans to address issues and opportunities to enhance member outcomes and overall well-being.
- Utilize clinical devices and data/information review to conduct comprehensive assessments and determine the approach to case resolution.
- Apply clinical judgment to integrate strategies aimed at reducing risk factors and addressing complex health and social determinants that impact care planning.
- Perform crisis intervention with members experiencing a social health or clinical crisis and refer them to appropriate clinical providers.
- Provide crisis follow-up to ensure members receive suitable treatment/services.
- Collaborate with interdisciplinary care teams to achieve optimal outcomes.
- Identify and escalate quality of care issues through established channels.
- Utilize influencing/motivational interviewing skills to ensure maximum member engagement and recognize their health status and needs.
- Provide education, information, and support to empower members to make informed clinical and lifestyle decisions.
Essential Qualifications:
- Current unencumbered Oklahoma state licensure as an RN.
- 3+ years of clinical practice experience (e.g., hospital setting or alternative care setting).
- Completion of Social Health Caseworker certification training provided through the Department of Mental Health and Substance Abuse Services within six months of hire date.
- 2+ years of experience using PC, keyboard navigation, navigating multiple systems and applications, and utilizing MS Office Suite applications.
- Ability to travel in-state up to 30% of the time, with mileage reimbursement per company policy.
- Willingness to provide on-call CM coverage for evenings and weekends on a rotational schedule as required by the State regulatory agency.
Preferred Qualifications:
- Experience caring for Native American/American Indian/Alaskan Native populations.
- Case management in an integrated model preferred.
- Managed care/utilization review experience preferred.
- Discharge planning experience preferred.
- BSN degree preferred.
