Key Responsibilities: Delivering Exceptional Patient Care
- Verify the plan of care with the RN prior to each visit, ensuring that patient needs are met and care is coordinated.
- Collaborate with physicians and RNs to ensure patient needs are being met, notifying them promptly of any changes in patient condition.
- Teach patients and caregivers self-care techniques, promoting independence and confidence in managing their healthcare.
- Check vital signs and review medications, performing medication reconciliation at each visit to ensure accuracy and safety.
- Perform specialized procedures ordered by physicians, such as foley catheter changes, using aseptic technique to minimize risk.
- Prepare equipment and materials for treatments, maintaining a safe and efficient care environment.
- Complete documentation timely and accurately, ensuring that patient records are up-to-date and reflect the care provided.
Qualifications: Joining Our Team
To be considered for this role, you must have:
- One year of clinical LPN experience, demonstrating your ability to provide high-quality patient care.
- A current LPN license, specific to the state(s) you are assigned to work, ensuring that you're qualified to practice in your assigned location.
- Current CPR certification, reflecting your commitment to staying up-to-date with the latest life-saving techniques.
- A valid driver's license, reliable transportation, and liability insurance, enabling you to travel to patient visits safely and efficiently.

Compensation and Benefits: A Comprehensive Package
We offer a competitive salary, ranging from $23.00 to $30.00 per hour, depending on your location, experience, and qualifications. In addition to your salary, you'll enjoy a comprehensive benefits package, including:
