Key Responsibilities:

• Conduct comprehensive home health assessments on pediatric patients, from newborns to teenagers
• Develop and implement individualized care plans, in collaboration with the patient's family and healthcare team
• Provide skilled nursing care, including medication management, wound care, and treatment of various medical conditions
• Educate patients and families on self-care techniques, disease management, and prevention strategies
• Monitor patient compliance and adjust care plans as needed
• Perform various treatments, such as NG or GTube feedings and ostomy care
• Maintain accurate and detailed records of patient care, using electronic charting tools
• Collaborate with the patient's healthcare team to ensure seamless care coordination

Requirements:

• Current and unencumbered state licensure as a Registered Nurse
• Minimum 1 year of recent clinical nursing practice, preferably in ICU or pediatrics
• Demonstrated passion for working in pediatrics and providing high-quality care to children
• Experience with NG or GTube feedings and ostomy care is preferred
• Prior home health experience is required
• Strong communication and interpersonal skills, with ability to work effectively with patients, families, and healthcare teams
