Responsibilities:
- Perform patient assessments and collaborate with the care team to develop and implement a plan of care
- Promote patient health and independence through teaching and appropriate rehabilitative measures
- Supervise LPNs and HHAs, complete documentation timely and accurately, and regularly communicate patient progress to the clinical manager and care team
- Participate in clinical development and continuing education programs, and perform on-call responsibilities and on-call services to patients/families as assigned
Qualifications:
- One (1+) year of clinical experience as a Registered Nurse (RN)
- Current RN license, specific to the state(s) you are assigned to work
- Current CPR certification, valid driver's license, reliable transportation, and liability insurance
